Membership Forms
Step 1. Provisional Member Form: Please fill this form out and submit it to one of the ODE board members.
Step 2. Service Form: Bring this form each time you earn credit towards ODE membership. This serves as proof that a fulfilled point(s) was earned.
Step 3. Application to ODE: Finally, after you have completed all the required points, submit this form to an ODE executive board member. Applications are reviewed by the faculty advisor.
