STATION forms

SBU-TV offers access to all aspects of television production. To assure the safety of our equipment and adherence to our practices we ask you to look over our organization constitution and our station forms. Each is available to print and at SBU-TV offices upon request. Forms are required to be filled out correctly before equipment is allowed to be released.

SBU-TV CLUB CONSTITUTION   (covers all bylaws and practices the station abides)

EQUIPMENT REQUEST FORM   (to request SBU-TV equipment for a show)

SHOW PROPOSAL FORM   (to propose a programming idea to the Executive Board)

STUDIO REQUEST FORM   (to request SBU-TV studio space)

PROGRAM RELEASE FORM   (to grant SBU-TV permission to broadcast your show)

LOCATION RELEASE FORM   (to give SBU-TV permission to film in a particular location)

TALENT RELEASE FORM   (to give individuals the right to be filmed for an SBU-TV production)

BULLETIN BOARD ANNOUNCEMENT REQUEST FORM   (to submit an announcement to be aired on SBU-TV's televised bulletin board)

PERSONAL INFORMATION FORM   (submit if you want to get involved with SBU-TV)
